Why Parallel Links?

Many primary care practices struggle to accommodate time for complex care.  Our youth are more stressed than ever before.  There are a plethora of reasons including: 

social media, school stress, socioeconomic stress, time constraints leading to inadequate sleep and nutrition, lack of resiliency, poor self-esteem, less emphasis on spirituality and nature, a pressure to conform, substance use, fewer resources for chronic pain and mental health, less emphasis on life skills, insufficient education for providers, parents and teachers regarding mental health and neuroatypical youth, fewer resources for services....

The list can go on.  

And we can help our kids find strength and resiliency!

I know  I need time to really assess the big picture, working together to make a plan that may include life skills, family dynamics, education on nutrition, sleep and ways to motivate, even bringing spirituality and art into treatment on an individual level.  This work demands extended time to assess emotional, medical, learning, and general health needs as a group to form the best treatment plan.

When Dr Donnesha Slider asked me to join her at The Nest, I knew this was a chance to practice the way I could help families the most.

About Dr G

I have practiced pediatrics for 25 years in the Pittsburgh area.  My love of pediatrics has always been based on working with families and making sure every patient has their chance to be the best they can be when negotiating the adult world.

My focus is on healthy lifestyle, behavioral health of child and caregiver, complex medical care, teen health, and transition to adult care.  I began advanced training in behavioral health and transition to adult care over 10 years ago.  I currently work closely with psychiatrists, psychologists, pediatric specialists, life coaches, and educators.  My practice evaluates and treats mental health concerns, behavioral concerns, teen health concerns, and pediatric medical issues.  Given my co-location with Manifest Psychiatry, I have ongoing psychiatric collaboration to help determine level of care or fine tune medicine management.  Patients can even move to psychiatric services if needed and return once medications are stabilized.  We have found this collaboration of Pediatrics and Psychiatry to be a powerful way to get kids better faster physically and emotionally.

If you have concerns about your child, whether it be behavior in early school years or a withdrawing teen, I am here to help.  I will assess for concerns such as anxiety, depression, ADHD, and autism.  In addition, I will assess complicating medical concerns such as headache/stomachache/other physical symptoms.  Often brain and body concerns link and I will work with you and your child to determine this.  If indicated, labs and medications will be discussed and ordered.  I collaborate with schools, teachers, and community therapists to help make a coordinated plan forward.  Our goal is reaching full potential!!

Dr Gotkiewicz sees children, teens, and young adults through age 24

Visits are extended time compared to traditional pediatrics.  We will thoroughly discuss your concerns and evaluate functioning in school, socially, and at home.  Lifestyle dynamics are discussed as well as the importance of sleep, nutrition, and self-care.  Evaluation and treatment of medical conditions that may contribute to mental health is available.   Concerns for anxiety, depression, ADHD, autism, or other behavioral health issues will be evaluated and discussed.  Options for treatment and/or referral for clarification are available.  Treatment plans may include counseling referral, lifestyle goals with patient/family education and encouragement, laboratory workup, medication initiation and management, and clear explanation for plan forward.  Letters for IEP/504 and collaboration with schools is available if deemed appropriate.

Parallel Links does not provide primary care services.  The intent is to work with your PCP.  However,  pediatric offices have varied levels of comfort with mental health and often have limited ability to offer extensive visits.   Your plan will be individualized based on need and options available.

Full Assessment:  medical and mental health needs plus wellness

This is the majority of initial visits!!!!!

May include time for record review, family history, behavioral health evaluation, medical evaluation, communication with pediatrician, psychiatry, school and other supports, laboratory tests, specialist referrals, education on diagnosis, medication, lifestyle options and plan forward.

Cost:  $450 (can be done in two 60 min visits, $225 each)

Full Assessment High Complexity:  medical and mental health needs plus wellness

     May include time for record review, family history, behavioral health evaluation, medical evaluation, communication with pediatrician, psychiatry, school and other supports, laboratory tests, specialist referrals, education on diagnosis, medication, lifestyle options and plan forward.  up to 3 hour evaluation.

 

Cost:  $600 (can be done in two 90 min visits, $300 each)

Prices are based on pediatric medical codes.
if additional time is needed for work outside of visit, it will be discussed with patient/family.  We will do our best to keep costs to those listed unless otherwise requested.

Parallel Links is currently working on insurance panel options.  For now we ask for payment at time of service and will be considered out of network.
You should check with your individual insurance regarding out of network costs.

         Dr Slider is co-located with me at The Nest.  We can do a verbal communication about your case ($50), offer a blended membership plan allowing in person consultation during your regular visit when needed (see below), or even transfer care depending on needs.  

 Transition to College with Dr G      always a favorite!!!!

College is a huge change for our kids.  I want my patients to be fully educated on wellness, resilience, and self-efficacy.  I educate on sleep and why it helps your brain, proper body fueling, ways to stay active and make time for self, social stressors, relationships, sexual health, and safety.  

For those with ADHD, anxiety, depression, or other mental health diagnoses I discuss safety with medications, recognition of a problem, ways to get help, and how to work with disability resources for accommodations if needed.  

With our membership tier 1 plan, I can follow your student over 5 visits during the year to check in on performance and wellness.

Cost: 

General education  (60 minutes with materials provided)    $200

General education + establish care (uncomplicated) (90 min)       $300

General education + establish care (requesting medication or current medications need assessment) (needs full assessment, see above, once stable will qualify for membership)

Medication Management during College:  establish care with full or targeted assessment then if deemed appropriate choose membership (targeted check in times) or standard care.  Frequency of follow up is determined individually depending on stability.  Refills will require appropriate follow up.  Video visit options available after initial visit depending on location.

For example, anxiety that is stable may require check in 4 times a year for med refills, over 18 and after freshman year college may be twice a year

I feel very strongly that mental and physical health is evaluated with enough time to make sound decisions and help the family and patient understand their care.  Insurances do not always offer options for extended time appointments.  I am working hard to provide insurance coverage and will update if this changes.

Do not worry! Many plans offer out of network coverage (flex/HSA/etc).  I will also work with you to make this affordable and great quality care.  

Out of Network Reimbursement

Prior to starting care we encourage you to contact your insurance provider by calling the number on the back of your insurance card. Please ask about your out of network benefits. We will provide all billing and insurance required documents to allow you to submit claims for out of network benefits. You can submit this to your insurance company. Your insurance company can then reimburse you for your out of network benefits directly after you submit the claims for services that you pay for by credit card and/or flexible health care spending account. We keep credit cards on file within the medical record system to ensure it is secure. We also utilize a patient portal to allow you to access your medical information at any time and communicate with Dr. G.
